Your general practitioner will usually arrange an appointment for you in advance. Confirmation of attendance can be made by telephone, e-mail below or letter. Fees for each consultation and reviews are payable on the day you attend. These fees may be reclaimed from VHI, Quinn, etc. Please contact your health insurer for details. Credit/laser cards are accepted.
Please bring any x-rays, MRI or other imaging films with you on your consultation date. Also, please bring a completed Patient Consultation Details form (see link below).
If for any reason you will not be attending for your appointment, please contact the clinic at least 48 hours in advance on 021-4305037.
The following forms can be downloaded to your computer and printed at home. By filling out this form ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out this form, please do the best you can and our staff will assist you with your questions on the day of your visit.
